sporadisk clearcelle-RCC er karakteriseret ved en defekt i tumorsuppressor-genet von Hippel-Lindau (VHL).<\/p>\n<p>VHL-proteinet spiller en vigtig rolle i reguleringen af det cellul\u00e6re respons p\u00e5 hypoxi via dets regulering af hypoxia-inducible factor (HIF), som en transkriptionsfaktor.<\/p>\n<p>Inaktivering af VHL-genet imiterer en hypoxisk tilstand, som for\u00e5rsager ophobning af HIF, hvilket \u00f8ger produktionen af forskellige v\u00e6kstfaktorer inklusiv VEGF (vascular endothelial growth factor) PDGF (platelet derived growth factor) og TGF-\u03b1 (transforming growth factor alpha), som stimulerer cellev\u00e6kst samt dannelse af nye blodkar. Dannelse af nye blodkar kaldes ogs\u00e5 angiogenese, hvilket spiller en vigtig rolle i patogenesen af RCC.<sup>1<\/sup><\/p>\n<p>V\u00e6kstfaktorer, s\u00e5som VEGF og PDGF-\u03b2, binder til deres respektive receptorer p\u00e5 endotelceller og kr\u00e6ftceller, hvilket aktiverer tyrosinkinasen p\u00e5 den intracellul\u00e6re del af receptoren, som igen resulterer i aktivering af intracellul\u00e6re signaltransduktionsveje, herunder signalvejene RAF\/MEK\/ERK og PI3K\/AKT\/mTOR. Disse signalveje p\u00e5virker igen celleadf\u00e6rden inklusive cellev\u00e6kst og angiogenesen.
